Cash Pay vs Insurance: When Self-Pay Makes Sense

Paying cash for imaging can sometimes cost less than using insurance. Learn when self-pay pricing is the smarter financial choice.

It sounds counterintuitive: why would you pay cash for a medical scan when you have insurance? But for many patients, self-pay pricing can actually cost less than going through their insurance plan. Here is when it makes sense.

When Cash Pay Beats Insurance

There are several situations where paying out of pocket is the better deal:

1. You Have a High Deductible

If your annual deductible is $2,000 to $5,000 and you have not met it yet, your insurance will not pay anything until you reach that threshold. You are responsible for the full allowed amount — which can be higher than a cash price at a freestanding center.

Example: Your insurance allowed amount for an MRI is $800. Your deductible is $3,000 and you have only used $200. You owe the full $800. Meanwhile, the cash price at a freestanding center might be $400 to $500.

2. You Need the Scan Fast

Insurance pre-authorization can take 3 to 5 business days — sometimes longer. If you need answers quickly, paying cash lets you schedule immediately without waiting for approval.

3. You Do Not Have Insurance

About 27 million Americans are uninsured, according to the U.S. Census Bureau. Self-pay pricing gives you access to the same high-quality imaging at a transparent, affordable rate.

4. Your Plan Has Limited Imaging Coverage

Some insurance plans have caps on imaging benefits, exclude certain scan types, or require you to use specific facilities. Cash pay gives you the freedom to choose where and when you get your scan.

How Much Can You Save?

The price difference between hospital radiology and a freestanding imaging center can be dramatic. For example, a hospital MRI without contrast might cost $1,500 to $3,000, while the same scan at a freestanding center could be $400 to $700. CT scans, mammograms, and ultrasounds show similar differences.

At AMI in Seminole, FL, our self-pay rates are among the most competitive in Pinellas County.

Does Cash Pay Affect Your Insurance?

Paying cash for one scan does not cancel your insurance or affect your coverage for other services. However, keep in mind:

  • Cash payments may not count toward your deductible — Ask your insurer
  • You can still submit the claim — Some insurers will apply the payment to your deductible if you submit the receipt
  • Get an itemized receipt — You may need it for your HSA, FSA, or tax records

What About HSA and FSA?

Yes, you can use your Health Savings Account (HSA) or Flexible Spending Account (FSA) to pay for imaging — even at cash-pay rates. These accounts use pre-tax dollars, giving you an additional 20-30% savings depending on your tax bracket.

How to Decide: Cash vs Insurance

Ask yourself these questions:

  • Have I met my deductible this year?
  • What is my allowed amount for this scan?
  • Is pre-authorization required, and can I wait?
  • What is the cash price at the facility?

If the cash price is lower than your out-of-pocket cost through insurance, self-pay is the obvious choice.
